Must be 4 years old by August 1, 2019 (or 3 by August 1, 2019 if you are attending 3 year old program)

Documents Needed

Completed Application
Copy of Birth Certificate
Copy of Shot immunization
Copy of Social Security Card
Proof of Residency
- Such as a Utility bill.
Proof of Income
- Please provide one of the following for ALL caregivers
  - Proof of income (2018 Taxes)
  - Months’ Worth of Paystubs
  - Notarized letter stating you are unemployed and earn no income at this time.
