Completed Application

  • Copy of Birth Certificate

  • Copy of Shot immunization

  • Copy of Social Security Card

  • Proof of Residency

    • Such as a Utility bill.

  • Proof of Income

    • Please provide one of the following for ALL caregivers

      • Proof of income (2025 Taxes)

      • Months’ Worth of Paystubs

      • Notarized letter stating you are unemployed and earn no income at this time.

      • Well Child Check
